Membership

Eligibility for Membership

Any person able to document his/her lineal descent from one or more of the following Mayflower passengers is eligible to apply for membership in the Societyof Mayflower Descendants.

The quickest way to demonstrate bloodline descent is through a prior application of someone who is related to you. If the relative is not from Arizona you can obtain a copy of an approved application for a $10 fee from the General Society of Mayflower Descendants, P.O. Box 3297, Plymouth MA 02361. You can link an approved application to you through the addition of your own proofs, i.e., births, marriages, and deaths.

Initial Adult Application

Print out and complete the New Member Preliminary Application. Make a copy for yourself and send the completed application along with a self-addressed stamped-envelope and a $5 check processing fee to the Arizona Membership Chairman, Shanna Gallo, 3443 E. Glenhaven Drive, Phoenix, AZ 85048. When submitting the New Member Preliminary Application you should attach a lineage outline or application review form that shows your line of descent. The National By-Laws require state sponsorships. We will assist you to find sponsors if you do not know any members of the Arizona Society. The Arizona Membership Chairman will then send you a Worksheet. Upon completion of the Worksheet and obtaining supporting documentation for each entry, send the Worksheet and two copies of each piece of documentation to the Arizona Historian, Bonnie McCulley, 1902 E Tamar Rd, Phoenix, AZ 85086-8484, with a check for $108 for the initiation fee which includes the first year's dues of $28. Dues included with the initiation fee apply to the calendar year in which the applicant's membership is finally approved by the General Society Historian, except that if such date falls after October 1, the dues will be allocated to the following calendar year. If the applicant's lineage line connects with the line of a former or current member of the Mayflower Society, the fore going procedures often can be shortened.

Junior Membership Application

A family member may sponsor for junior membership any individual under 18 years of age who is of the same family and bloodline as the sponsor and has a prima facie proven lineage. An application form may be obtained from the Junior Membership Chairman, Diane Moore, 8054 E. Monte Avenue, Mesa, AZ 85209. The completed application form is to be accompanied by a one-time fee of $15 and a copy of the junior's long-form birth certificate. Junior memberships expire at the age of 25, but a junior may apply for adult membership after the age of 18.

Annual Adult Dues

Annual dues are $28 (General Society $11 and the Arizona Society $17). Dues are payable upon receipt of billing in January each year.
